Proxy form:  If you need to appoint a proxy for a meeting, then this form needs to be in the hands of the secretary before the meeting begins. 

Nomination Form:  Use this to nominate members for the position of office bearers and committee of management. This form must be in the hands of the returning officer fourteen (14) clear days before the annual general meeting.
